New Client Cards
New Client Cards! ARTDOCS has a new sign-up process for all patients. In November of 2010, we mailed letters to our client base about how to update eligibility for ARTDOCS coverage and receive a laminated client card. Call (504)827-2833 for an appointment to come to our main office in Mid-City at 2646 Dumaine Street. Clients will need to bring an ID with a local address, financial data (eg, pay stub, artist contract, IRS form), and some indication of their art (eg, postcard from a gallery showing, write-up in a newspaper/magazine.) Clients will fill out a brief contact form and intake questionnaire.
